What immunizations are required for International Students?

There are no required immunizations for International Students (except those in health related schools/programs). However, all students are required to fill out the Immunization form with their history of vaccinations already taken.

What immunizations are required for Undergraduate Students?

There are no required immunizations for Undergraduate Students (except Nursing Students). However, all students are required to fill out the Immunization form with their history of vaccinations already taken.

Undergraduates may upload immunization records so that they have access to the data if needed (e.g. for travel or volunteer work).

What immunizations are required for Graduate Students?

There are no required immunizations for Graduate Students (except those in health related schools/programs). However, all students are required to fill out the Immunization form with their history of vaccinations already taken.

What immunizations are required for Nursing Students (BSN and MSN)?
  • Hepatitis B vaccines: 3 doses required or positive Hepatitis B titer.
  • Tdap (Tetanus, Diphtheria, Pertussis): One dose of Tdap vaccine on or after age 10. If Tdap was given more than 10 years ago, a recent Td booster is required.
  • MMR (Measles, Mumps, Rubella) vaccines: 2 doses after 1 year of birth; minimum 4 weeks apart or positive Rubeola, Rubella and Mumps titers.
  • Varicella vaccines: 2 doses; minimum 4 weeks apart or positive Varicella titer.
  • Influenza vaccine: required during each flu season.
  • Tuberculosis Skin Test (PPD): refer to Tuberculosis FAQ. If TB testing was done and was positive, a chest x-ray is required and report needs to be submitted. This can be arranged by University Health Service.

What immunizations are required for Physician Assistant Students?

​​Proof of immunization ​​and titer for MMR, Varicella, and Hepatitis B are required. Should the titer be negative, booster doses will be required without followup titer.

  • Tdap (Tetanus, Diphtheria, Pertussis): One dose of Tdap vaccine on or after age 10. If Tdap was given more than 10 years ago, a recent Td booster is required.

If proof of primary series cannot be furnished, then a titer will suffice. Proof of vaccination alone is NOT sufficient.

What immunizations are required for Public Health Nutrition Dietetic Internship students?
  • Hepatitis B vaccines: 3 doses required or positive Hepatitis B titer.
  • Tdap (Tetanus, Diphtheria, Pertussis): One dose of Tdap vaccine on or after age 10. If Tdap was given more than 10 years ago, a recent Td booster is required.
  • MMR (Measles, Mumps, Rubella) vaccines: 2 doses after 1 year of birth; minimum 4 weeks apart and positive Rubeola, Rubella and Mumps titers.
  • Varicella vaccines: 2 doses; minimum 4 weeks apart or positive Varicella titer.
  • Influenza vaccine: required during each flu season.
  • Tuberculosis Skin Test (PPD): refer to Tuberculosis FAQ. If TB testing was done and was positive, a chest x-ray is required and report needs to be submitted. This can be arranged by University Health Services. Please note that some clinical sites require that PPD test is done by their approved agency regardless of the last date of your PPD.
  • Physical exam: within the last 12 months by physician or mid-level provider. If necessary, student can schedule physical exam with University Health Services. Must be completed by October 15.
  • Urine screen: schedule with University Health Services during the month of September.

What is a "titer"?

A titer is a blood test that confirms immunity.

I can't find my immunization records; do I need to have my shots again?

You should contact your Health provider who can provide updated vaccinations or perform a test that will check your blood for antibodies which will determine immunity. You can also have these tests done at UHS at your own expense.

How much do immunizations cost at UHS?

Students with Case Medical Plan are fully covered for required immunizations. For those students that are not using Case Medical Plan, please check with your health insurance carrier to see what vaccines are covered and how you can be reimbursed in the event that you obtain the immunizations at UHS. A list of available vaccines and costs can be found on our website.

I made a mistake on my online form, what should I do?

You should alert the University Health Service that the information you submitted is different than the information on your documentation. Your healthcare provider's documentation is the document of record.

How do I submit my immunization records so they will be added to my campus health record?

You should upload your record to by logging onto myhealthconnect.case.edu and selecting Immunization Upload. This method is secure and places your information directly into your chart.

What if I just send in my documentation, but don't fill out the online forms?

Filling out the online forms is required. Your documentation will not be processed unless you enter the information online and then submit the documentation. If you cannot access the online form or need to resend the documentation, please contact us.

What if I fill in the online form but don't submit any documentation?

Your online information must be verified by the supporting documentation before it can be considered part of your medical record. Health-related students are required to provide documentation of their immunization records.

What if I do not have all the required immunizations by the deadline?

Submit your immunization form by June 30 with your current information. Then, contact us to let us know that you plan to get any additional required vaccines before coming to campus. If you get a vaccine after submitting your form, contact us to update your information.

If I have no opportunity to get a required vaccine before coming to campus, what are my options?

If you need to receive a vaccine at UHS, you can do so when you arrive on campus. For students on the Case Student Medical Plan, there is no charge for required vaccines. For students on other insurance plans, please check our website or contact us at HealthService@case.edu.

The University Health Service is open 8:30 a.m. to 4:30 p.m. Monday, Tuesday, Wednesday and Friday, 9:30 a.m. to 4:30 p.m. Thursdays.
