Form 1095-B 2015 tax document

The Affordable Care Act (ACA), also called the health care reform law, requires every person to have basic health insurance or face a penalty. CWRU is committed to providing you and your family with affordable, substantial health benefits that meets the requirements of "minimum essential coverage" under the ACA.
The Internal Revenue Service (IRS) requires us to report those people that are covered. Form 1095-B will be mailed to you as proof of the health coverage provided under the Student Medical Plan, as well as your dependents if they were enrolled in the Optional Dependent Plan for all or part of the 2015 tax year.
You can use Form 1095-B to help report your insurance coverage when filing your tax returns. Only one form is provided to all the individuals listed on your policy.
